This service is intended only for individuals who no longer have access to their original course completion certificate(s) in the Learning Center.
Eligibility: Certificate reissue requests are available only for courses completed within the past five (5) years.
If you took a course within the last 6 months, your certificate(s) should still be available in your Learning Center – do not purchase this service. You may download them at no additional cost.
Course Certificate Reissue Request
Purchase this service to request replacement copies of previously earned Academy of Pelvic Health Physical Therapy course completion certificates within the last 5 years.
How It Works
- Purchase the Replacement Course Certificate Request.
- Complete the Replacement Certificate Request Form (link provided after purchase).
- Follow the instructions listed. You are required to do a separate entry for each certificate you would like reissued.
- Academy staff will verify your course completions and email your replacement certificates.
Important Information
- The $5 fee covers one certificate reissue request, regardless of the number of certificates requested.
- Only certificates for courses completed within the past five (5) years are eligible for reissue. No exceptions will be made.
- This service is only for certificates that have already been earned.
- If your certificates are still available in your Learning Center (within the last 6 months), you may download them at no cost.
