For companies, employers, or attendees paying by check
Who can use this form?
- Employers paying for their employee's course registrations
- Companies paying for sponsorship or advertising
- Affiliates paying quarterly or annual fees
Step 1: Generate a Check
Generate a check and make sure it is made out to the following name:
Academy of Pelvic Health Physical Therapy
Step 2: Mail the Check Along with Invoices
Print out all associated invoices and place them in a standard envelope along with your generated check.
Please note that you are responsible for ensuring the check is delivered BEFORE at least 1 week before the course date. Course registration is not guaranteed without a valid payment method. If we do not receive your check at least 1 week prior to the course, the registration will be canceled. All course registrations must be fully paid by the time of the course.
